Position Summary

The Care Coordination Manager (CCM) is responsible for performance oversight, training, and caseload support for care management staff. The CCM partners with site managers by providing clinical and program specific feedback to supplement the day-to-day support of the site managers.? The CCM , working with CM leadership, develops tools and systems to efficiently measure performance, to analyze performance trends, and to innovate to better meet the needs of our patients. In collaboration with the Clinical Complex Care Director, the CCM supports the clinical and professional development of Care Managers.? 

Principal Duties and Responsibilities*

Training & Support

• • Coordinates onboarding and initial training for Care Managers 
  • Provide support to care management staff related to care management workflow, performance, and documentation expectations. 
  • Collaborates with other CCM and CM director on program vision, implements vision and provides quality oversight and support to CM staff 
  • Works with clinic site managers to assist in maintaining and improving performance of care management staff (e.g., providing consistent progress reports to CMs and their site managers, assisting site managers in understanding the role(s) of care management staff, and working with site managers to coach care management staff on day-to-day issues.) 
  • Works directly with care management staff to answer questions or troubleshoot issues related to performance metrics and/or workflow. 
  • Conducts monthly rounding sessions with assigned care management staff 

Monitoring and Analysis

• Develops tools, in collaboration with others, to measure performance for care management staff. 
• Regularly monitors care management staff performance, including caseload volume, timely completion of expected tasks, appropriate outreach and enrollment, productivity and compliance, etc. 
• Collaborates to ensure the care management team is meeting expectations of external partners and funders, as appropriate. 

Administrative

• Responsible for attending funder meetings and audits as necessary to communicate current performance on key metrics. 
• Communicates performance data regularly to all care management staff at CM meetings and individually at monthly roundings 
• Works consistently with other CCM and manager to divide work evenly and covers tasks when necessary 
• Responsible for upkeep of content-related resources, such as Wellness West SDOH referrals, or New Birth Boutique 
• Assists director in developing and honing protocols and policies related to population specific workflows

Required Skills or Abilities* 

• • Requires a comprehensive understanding of health care issues involved in providing clinical leadership at a Community Health Center. 
  • Requires experience and demonstrated abilities for working in a multi-cultural setting. 
  • Ability to strongly embrace and personify the mission of the Lawndale Christian Health Center, through performing work in a whole person way, providing for the physical, emotional, and spiritual needs of staff members and patients. 
  • Ability to collaborate and interact with a diverse group of health care professionals and synthesize ideas from different departments including operations, grants and analytics 
  • Ability to multitask, organize, direct, prioritize, and delegate work appropriately. 
  • Excellent analytical skills necessary for preparing financial, legal, and administrative tasks. 
  • Excellent consumer relations skills 

Required Knowledge, Experience, or Licensure/Registration

• Bachelors degree with significant case management experience or MSW, MPH, MHA, or related graduate degree preferred. 
• Preferred content expert in: maternal child health, chronic disease management or complex care
