The Care Coordination Program Manager oversees the day-to-day operations of the Care Coordination Program at Chapa-De including supervision of all department members, monitoring program goals/objectives, and meeting regulatory requirements/grant goals. This role will lead the development and continuation of the Enhanced Care Management Program and will oversee the transition of the Community Health Representative program into a billable service under CalAIM.
