    The Care Coordination Program Manager oversees the day-to-day operations of the Care Coordination Program at Chapa-De including supervision of all department members, monitoring program goals/objectives, and meeting regulatory requirements/grant goals. This role will lead the development and continuation of the Enhanced Care Management Program and will oversee the transition of the Community Health Representative program into a billable service under CalAIM.

    CARE COORDINATION PROGRAM MANAGER DUTIES:

    • Oversee Enhanced Care Management, Community Health Representation, Patient Care Coordination, Purchased/Referred Care and insurance eligibility services provided to Chapa-De patients.
    • Manage the implementation and continuation of applicable CalAIM initiatives (ex. Enhanced Care Management, Community Supports, and Population Health Management) to the care coordination department and serve as the primary contact with Medi-Cal Managed Care Plans for these initiatives.
    • Oversee and monitor community education, outreach and recruitment related to the Care Coordination Program.
    • Facilitate the design, development and implementation of processes, procedures and policies for the Care Coordination department.
    • Establish and monitor performance metrics with other members of the health care team to improve Care Coordination outcomes in medical, behavioral health and oral health within the organization.
    • Supervise the coordination of individual and group education for staff, patients, and relevant community partners on topics relevant to patient care coordination.
    • Develop, cultivate and/or strengthen community-based partnerships to reach disproportionately affected populations.

    Required Experience:


    MINIMUM QUALIFICATIONS:

    • BA/BS degree, preferably in social work; or equivalent combination of education and/or experience.
    • Two years’ experience in healthcare or health education program or department.
    • Minimum two years of experience supervising staff and effectively leading a work team.
    • Excellent communication skills (written, verbal and interpersonal).
    • Effective presentation skills.
    • Detail-oriented, with ability to establish timelines, meet deadlines, and manage budgets.
    • Computer proficiency in a variety of applications and programs including: Microsoft Office (Outlook, Excel, Word, PowerPoint, Teams) and Electronic Health Records.
    • Requires interest in the organization of health services delivery and sensitivity to the cultural and health needs of Native American Indian people.
    • Current CDL, proof of vehicle insurance, and DMV report.


    PREFERRED QUALIFICATIONS:


    • LCSW License, current and in good standing with the state of California.
    • Previous experience with managing grant requirements; Familiarity with grant funded
