    Overview:

    Health Assessment Nurse RN/LPN - Assisted Living, Full Time, M-F, 8 HR

    The Health Assessment RN/LPN is responsible for the well-being and nursing care of all residents assigned to them during their shift. You will provide nursing services following the patient’s treatment plan or physician’s directions, assess patients and respond to and report any changes in condition, distribute and chart medications, and respond to emergencies. Your professional knowledge and caring demeanor, coupled with a supportive staff and grateful residents, will make this a successful partnership.

    Some of the day-to-day responsibilities:

    • Assesses physical, spiritual, and psychosocial needs of residents & coordinates with consultants as needed;
    • Collects pertinent subjective and objective data regarding the health status of Assisted Living residents;
    • Delegates and monitors implementation of nursing process to appropriate staff;
    • Regularly involves the Assisted Living Team in evaluating resident function/ability, and keeps them informed of changes to Assessment & Service Plan;
    • Communicates in a timely manner significant changes in resident's status to appropriate members of the health care team;
    • Ensures new orders are transcribed correctly and necessary medications are requested from pharmacy in a timely manner;
    • Reports all incidents of abuse, suspected abuse, and negligence to the appropriate individual(s);
    • Participates in appropriate committees and collaborates with other staff /outside vendors;
    • Reports all incidents of abuse, suspected abuse, and negligence;
    • Monitors and responds to emergencies as delegated by Chief Healthcare Officer;
    • Participates in assessment, problem solving, resident care conferences, and discharge planning processes;
    • Supervises Certified Nursing Assistants.


    Qualifications

    • Degree from an accredited School of Nursing or Practical Nursing program
    • Current WA RN/LPN license
    • Current Basic Life Support (BLS) certification
    • Experience in electronic medical records preferred
    • One year of nursing experience (Preferably in Assisted Living or Geriatrics)
