This program gives consumers a safe, affordable place to live. FMRS staff provide support to maintain the community placement. These services are provided in a variety of settings including natural family settings, individual housing arrangements or housing for multiple consumers. Additionally, this program provides for transitional living/respite options with supportive services.
The primary services provided include practical help and supports needed to help consumers obtain the necessities of daily living, protective oversight and supervision. Additional services include the provision of direct services such as assisting consumers with grocery shopping or providing transportation to appointments. Attending to the basic needs of the consumers is an important function of the supported living program. These needs such as personal care, nutrition, housing and financial management, can be a good indicator of how well a person’s symptoms are being controlled or alleviated. All supportive living staff assess the consumer’s ability to perform their activities of daily living during their face-to-face and telephone contacts. When deficiencies are noted, the team determines a plan to address them.
FMRS has twenty-five supported housing slots. Two of these slots are normally utilized for transitional/respite housing. To be eligible for this program, consumers must have been involuntarily hospitalized and be at risk for additional hospitalization without the needed supports.
