Temp Social Work is currently seeking an experienced Case Manager (BSW) to fill an opening with an agency we are working with in the greater Marshall, MN area. The ideal candidates should be able to perform the duties of a CADI, Elderly Waiver, DD, BI, CAC, Case Manager and comfortable completing MNChoice assessments.

This position is open-ended and there is no known end date. The agency we are working with is open to some hybrid work.

Responsibilities of the Case Manager:

-Maintain caseload

-Develop and complete assessments

-Complete case records which meet federal, state and local guidelines

-Attend Court hearings as required

-Facilitate access to and refer clients to appropriate community services

-Prepares and maintains progress notes, narrative summaries, reports, and other required case documentation in the file and in SSIS

-Travel to meetings and clients homes as needed

Requirements of the Licensed Social Worker:
-Bachelor degree in Social Work from an accredited school

-Valid social work license in the state of Minnesota is preferred

-Experience working as a case manager

-Active MNChoice Assessment certification preferred

Job Types: Full-time, Temporary

Pay: $28.00 - $31.00 per hour
