Renaissance Nursing and Rehabilitation is seeking a full-time Social Worker for our skilled nursing facility located in Stattsburgh, NY. The ideal candidate will have experience in an LTC/Sub-Acute setting.

Duties Include:

• Conducting initial psychosocial assessment with resident upon admission
• Coordinating discharge planning for short term residents, actively participating in UR to scheduled and document discharge planning efforts and maintain facility discharge claendar
• Identifying individual social and emotional needs of residents, care planning for those needs
• Completing MDS releated assessments and completing social work sections of MDS
• Scheduling, documenting and coordinating resident care conferences
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• COVID-19 Vaccination per DOH requirements for SNF employees

Requirements:

• LMSW Preferred
• Previous Long-Term Care/Sub-Acute Experience Required
