Intake Coordinator conducts initial assessments of patients to determine mental health needs and establish care requirements. LOCATION: Thibodeaux- in home meetings in St. John and St. Charles Parish
Qualifications:
Bachelor's Degree in a human services field and experience working with infants and toddlers and/or school-age children with disabilities and their families. Knowledge of early intervention services, school system, health care, community resources, cultural competence and working with multi-disciplinary teams where families are equal participants is essential to the position.
Responsibilities:
1. Assist families from the point of referral, through the intake and eligibility determination process to the completion of the initial IFSP.
2. Upon receiving referrals from a parent, guardian, doctor, teacher, service provider, health or social services agencies, etc., Early Steps SPOE Intake Coordinator Specialist will:
a) Respond to any referral within two (2) business days either through a personal face to face interview telephone contact, or letter,
b) Make arrangements with the family for a face-to-face interview within ten(lO) business days of the date of referral if a personal face-to face interview is not conducted during the initial contact;
c) Conduct face-to-face interviews in the family's natural environment or document why this could not be done.
d) Introduce the family (during the initial face-to-face interview) to the Early Steps system and fully educate the family, in the family's primary language or other mode of communication (including in writing) about the family's procedural safeguards and right to due process.
e) Obtain informed written consent (after thorough review of the procedures and Safeguards) from the parent/legal guardian to participate in the Early Steps system in order to proceed with the intake process.
Utilize the LDH SPOE Manual as the primary reference for standardized consent forms and all other referral documentation forms for the Early Steps process.
3. Upon intake and after obtaining informed written consent, the SPOE Intake Coordinator Specialist will:
a) Initiate requested for existing information from appropriate health, medical, Educational, and other community services.
Utilize the LDH SPOE Manual as the primary reference for standardized intake forms, referral forms, eligibility form and IFSP.
b) Interview the family and obtain information from other sources regarding the referred child; (family interviews will include information about the child's and family's daily routines).
c) Review the child's situation as to whether a parent or guardian is present in order to determine if the referred child would benefit from a surrogate parent.
d) Complete and return to LDH within ten (10) working days of that child's appointment for the child believed to be eligible to receive a surrogate appointment.
e) Communicate with any/all referral sources within five (5) days of receipt of the referral was received, is in process, and advise the referral source of the next steps.
Upon Eligibility Determination, the Intake Coordinator Specialists will:
a) Fully advise and educate the parent/legal guardian about the family's procedural safeguards and right to due process regarding eligibility determination according to 34 CFR 303.400-460, and this will be done in the family's primary language or any other applicable method of communication (including in writing).
b) Obtain relevant LDH and federal releases of information and consents for additional eligibility determination activities.
c) Arrange or collect relevant assessments and evaluations (after receipt of parent/legal guardian authorization), coordinate a comprehensive developmental screening, and/or conduct comprehensive parent interviews to determine areas of delay or concerns;(assessments and evaluations will also address the child's and family's natural daily routines).
d) Assist families in choosing providers to perform multi-disciplinary assessment.
e) Have the child's multi-disciplinary team available to review the diagnostic materials and assessments and facilitate the determination of the child's eligibility for Early Steps services in accordance with Early Steps of IDEA and state and federal regulations. The multi-disciplinary team Early Steps makes the determination of eligibility.
4. For notifications of eligibility or ineligibility for Early Steps services to the parent/legal guardian of the child applying for Early Steps services, the Early Steps SPOE staff will follow the policies and procedures of LDH and Early Steps of IDEA and state and federal regulations that pertain to notification of eligibility, procedural safeguards, due process rights, and record keeping (hard copy electronic).
5. Organize initial IFSP after assisting family in choosing service providers to attend. Initial IFSP must take place within 45 days of referral date.
6. In the event that there is no availability of Family Support Coordinators in the area to provide service, the Intake Coordinator Specialist will be responsible for assisting families in that role until service becomes available.

The position will have a full caseload up to 50 and receive supervision from the Intake Coordinator/Supervisor. This position is a 40 hour per week position.
