Location: Grand Forks, ND
Duration: 3 months with possible extension
Shift: Days (40hrs a week)
Job Description:
The Intake Services Coordinator, under clinical supervision of a licensed social worker or licensed professional counselor, coordinates the referral development and pre-admission patient assessment process.
Essential Job Duties/Responsibilities:- Responds to intake calls and completes admission paperwork. Assessments will be completed by RN, LPC or LMSW.
- Facilitates groups, family and individual sessions and completing all necessary documentation.
- Schedules Unit assessments, meetings, and appointments as appropriate.
- Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, LUMS, alcohol screening tool(s) under authorization/supervision of RN, LPC or LMSW.
- Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities.
- Reviews newly admitted patient accounts for eligibility and authorization daily.
- Addresses issues in a timely manner including crisis and higher level of care referrals.
- Performs other duties as assigned/required by this position.
