Job Description- Intake Coordinator
Title: Intake Coordinator
Report to: Direct Supervisor
Description: The intake coordinator is responsible for all intakes that are received from the various referral sources. The intake coordinator ensures that all patients receive an initial intake assessment to determine the proper case management level. The intake coordinator is responsible for coordinating discharged patients from the inpatient setting by determining eligibility first and then coordinating with New Hope the appropriate level of group home placement.
Qualifications: Bachelor's Degree in Social Work, Psychology, Human Services or other related fields preferred; however, education may be substituted for over 2 years experience in case management. A minimum of 2 years of experience providing direct services to one or more of the following populations is required: people with behavioral/ mental illness or people in recovery from addiction. Must have excellent oral and written communication skills; good organizational skills and an effective time manager.
