Job Description- Intake Coordinator

Title: Intake Coordinator

Report to: Direct Supervisor

Description: The intake coordinator is responsible for all intakes that are received from the various referral sources. The intake coordinator ensures that all patients receive an initial intake assessment to determine the proper case management level. The intake coordinator is responsible for coordinating discharged patients from the inpatient setting by determining eligibility first and then coordinating with New Hope the appropriate level of group home placement.

Qualifications: Bachelor's Degree in Social Work, Psychology, Human Services or other related fields preferred; however, education may be substituted for over 2 years experience in case management. A minimum of 2 years of experience providing direct services to one or more of the following populations is required: people with behavioral/ mental illness or people in recovery from addiction. Must have excellent oral and written communication skills; good organizational skills and an effective time manager.

Responsibilities:

• Completes a needs assessment on patients that are received from various referral sources
• Ensures that all patients received through the referral process has been properly assessed to determine the appropriate level of care
• Daily and weekly documentation in the appropriate systems
• Ensure patient confidentiality by complying will HIPAA regulations
• Participate in weekly Interdisciplinary Team Meetings and provide feedback regarding patients intake
• Coordinate with the Care Coordination Assistant for proper transporting of patients to group homes as required
• Ensure that all group home admission documentation has been received for all patients being discharged from the hospital
• Ensure that documentation has been submitted to the housing coordinator
• Coordinate with the housing coordinator all group home admits/placements
• Ensures that hospital discharged patient prescriptions have been submitted to the pharmacy timely for prompt processing and group home delivery
• Reviews and educates the patient on program participation and compliance and ensures that all patients have agreed to and signed their contracts
• Transfers all intake patients to the direct supervisor for case management assignment
• Effectively communicate the intake process to every patient
• Coordinate an effective and smooth intake for all patients
• Ensure that patients do fall through the cracks by ensuring that all referrals are properly and timely processed
