    COMPENSATION


    This position has a salary range of $62,400.00 to $75,700.00 depending on years of clinical experience and bilingual (English/Spanish) language skills.

    JOB SUMMARY


    The Intake Coordinator is responsible for the initial screening and management of referrals to all SBS mental health and housing programs. Participates in weekly meetings tracking referrals, facilitates interviews, monitors waitlists, and completes all intake paperwork. 


    ESSENTIAL DUTIES / RESPONSIBILITIES / EXPECTATIONS 

    • Maintain active liaison with referral sources to identify and cultivate new referrals, strengthen and solidify relationships with established referral sources and provide information to the community regarding agency services and programs.
    • Keep referral sources appraised of program openings.
    • Receive and review referring documentation. Follow up with referral source to get more information as necessary.
    • Work with applicable SBS staff to complete intake process – inclusive of phone contact with referral source, faxing of necessary paperwork, checking Medi-Cal eligibility, etc.
    • Conduct initial intake screening on youth who are referred for mental health and housing services to determine:
    1. Medical necessity and severity of symptoms
    2. Confirm active Medi-Cal coverage
    3. Which SBS program or clinician to match youth with
    • Conduct Initial Psychosocial Assessment and Initial Treatment Plan with new clients as needed
    • Work to resolve any identified problems with Medi-Cal eligibility
    • Track the referral process to assure prompt entry into services
    • Develop and maintain waiting list for programs at capacity.
    • Attend Alameda County Child and Family Team Meetings (CFTs) to educate youth about SBS programs.
    • Provide Medi-Cal eligibility workshops for youth.
    • Attend and participate in all relevant supervisions, meetings and trainings
    • Open clients in Electronic Health Record, and update and maintain records as needed.
    • Coordinate and facilitate introduction meetings to ensure youth smoothly transition into housing program.
    • Maintain spreadsheets and documents necessary to organize program information.
    • Complete data reports and send to county as needed.
    • On Call responsibilities as needed

    Requirements

     QUALIFICATIONS

    • Masters in Social Work or Marriage and Family Therapy program from an accredited university or college.
    • At least two years of experience working with adolescents, preferably current and/or former foster youth
    • Fulfillment of TB clearance and any other mandatory state/federal requirements.
    • California Department of Justice and FBI fingerprint clearance
    • A valid California driver's license, dependable personal automobile for use on the job, insurable by SHS’ insurance policy based on DMV record and personal vehicle insurance.
