We are seeking a highly organized and detail-oriented Intake Nurse to join our team. The Intake Nurse will be responsible for facilitating the patient admission process for home health care. To ensure the provision of safe, quality, and cost effective referral processes. To ensure the process of referrals, physician orders and payor authorizations in a manner which conveys the philosophy, mission and vision of Panhandle Home Health, Inc. The ideal candidate will have excellent communication skills, and a thorough understanding of medical terminology and patient care.
Responsibilities:
- Demonstrate critical thinking and decision making abilities in receiving, tracking, documenting, and communicating with staff of patient referrals, physician orders and payor authorizations.
- Documents on all required forms and ensures confidentiality of all medical records.
- Provides coordination, referral development, consultation and education.
- Educate patients and their families on healthcare procedures, medications, and treatment options
- Maintain patient confidentiality in accordance with HIPAA regulations
Skills:
- Strong knowledge of medical terminology and medical records
- Excellent communication skills, both verbal and written
- Ability to work well in a team environment and collaborate with healthcare professionals
- Attention to detail and strong organizational skills
- Ability to prioritize tasks and manage time effectively
- Proficient in using electronic medical record systems
- Current RN or LPN license
