The Licensed Clinical Social Worker collaborates with the multi-disciplinary team to provide input in the development of the plan of care for those patients/clients requiring social work intervention. The Licensed Clinical Social Worker provides counseling and crisis intervention to patients/clients who have social or emotional problems in a variety of settings across the age span.
Essential Duties and Responsibilities:
- Provides necessary social work intervention
- Counsels individuals and families in crisis and help with issues such as poverty, loss and illness
- Assesses patient’s psychosocial needs, plans for, evaluates and modifies interventions to meet the established patient/client goals
- Collaborates with all disciplines to plan and evaluate team goals for each patient/client
- Facilitates patient/client and family referrals to community agencies when appropriate
- Provides financial counseling when necessary to achieve the treatment goals established by the multi- disciplinary team
- May be required to respond to crisis situations, including domestic violence and child abuse
- May be required to provide witness services and evidence in court cases
- Visits schools to assess academic concerns
- Works with government agencies to help clients apply for Medicare and Medicaid when appropriate
- Arranges home visits to evaluate a patient/client health, home environment and family interaction/support
- Assists patient/family/caregivers in planning for present and future psychosocial needs
- Observes and reports patient progress and/or changes in patient status to physician and appropriate multi- disciplinary team members
- Adheres to facility and Maxim policies and procedures at all times
Minimum Requirements:
- Master’s degree in Social Work from a school of social work accredited by the Council on Social Work Education required
- Active license as an LCSW in state of practice required
- Minimum of one year of social work experience preferred
- Complies with all relevant professional standards of practice
- Participation and completion of Maxim’s Competency program when applicable
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or state regulation)
- Must meet all federal, state and local requirements
- Successful completion of new hire training as applicable to job site
- Understand patient confidentiality and HIPAA requirements
- Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
- Computer proficiency required
- Must be at least 18 years of age
