As a Licensed Practical Nurse (LPN) you will be called to care when you’re needed most. As a member of our home health team, you’ll support a full range of patient services to bring comfort and dignity to our clients.

As a Licensed Practical Nurse (LPN), you will:

• Provides technical nursing services in accordance with agency procedure manual policies and Licensed Practical Nurse licensure regulations.
• Prepares clinical and progress notes.
• Assists the patient in learning appropriate self-care techniques.
• Confers with case manager or clinical supervisor regarding changes in patient’s condition and needs.

To qualify for a Licensed Practical Nurse (LPN), with us:

• Must have graduated from an approved school of Practical Nursing.
• Must be licensed to practice in the State of Indiana.
• Must have one year of nursing experience.
• Licensed Practical Nursing curriculum must have included a pharmacology course.
• Current CPR certification.
• Valid state-issued driver’s license, automobile insurance, and reliable transportation; willingness to travel.
