Licensed Practical Nurse (full-time)                 $19.24 - $24.72 / hour    

(Hourly rates based on qualifications and experience)

Position Summary:  To coordinate the patient centered medical home, collaborating with clinicians, nursing and support staff to help ensure the smooth progression and throughput of daily patient schedules.  Act as a liaison between clinics, hospitals and community resources helping patients receive prompt access and continuity of care. Foster a culture of patient and family centered care, service, quality and safety excellence, while improving patient experience, employee experience and community relations.
