    SUMMARY
    Serves as a liaison between the professional staff, community, hospital, healthcare facilities,
    physician, and physician's staff. Acts as a resource for information to ensure quality care for the
    patient.


    ESSENTIAL DUTIES AND RESPONSIBILITIES:
    1. Complies with agency policies and procedures.
    2. Understands and effectively implements the selling strategies of the VNA, shows the
    ability to resolve issues effectively, follows up with Physicians and Physicians staff on
    status of their patients. Attains admissions goals set by management.
    3. Ensures open line of communication and education between the agency, Physician,
    Physician's staff, and hospital staff.
    4. Serves as a liaison between the Agency and the medical community. Conducts medical
    community awareness to promote services of the agency. Makes a minimum of ten calls
    per day on accounts assigned in your territory and input calls into tablet daily.
    5. Keeps physician profiles current. Provides documentation/information to Director of
    Nursing, Supervisors and clinical staff to facilitate quality patient care, and track
    referrals.
    6. Retrieves signed POCs or Physician Change Orders from physician's offices as requested.
    Notifies Director of Nursing immediately if there are problems obtaining signed
    POCs/VOs.
    7. Participates in staff development activities and in-service meeting as required.
    Represents the agency at all times in a professional and loyal manner. Represents the
    agency at various community network meetings.
    8. Maintains client/patient confidentiality in accordance with Florida Law and the Health
    Insurance Portability Accountability Act (HIPAA).
    9. Performs other duties as assigned.


    QUALIFICATIONS

    EDUCATION and/or EXPERIENCE
    Requires a college degree with study emphasis in communication, public relations or related field. Suitable experience may substitute. Prefer knowledge of the home health industry and Medicare guidelines for home health.


    LANGUAGE SKILLS

    Ability to read, analyze, and interpret common scientific and technical journals, financial reports,
    and legal documents. Ability to respond to common inquiries or complaints from customers,
    regulatory agencies, or members of the business community. Ability to write speeches and
    articles for publication that conform to prescribed style and format. Ability to effectively present
    information to top management, public groups, and/or boards of directors.


    MATHEMATICAL SKILLS
    Ability to calculate figures and amounts such as discounts, interest, commissions, proportions,
    percentages. Ability to apply concepts of basic algebra.


    REASONING ABILITY
    Ability to define problems, collect data establish facts, and draw valid conclusions. Ability to
    interpret an extensive variety of technical instructions in diagram form and deal with several abstract and concrete variables.
