Social Worker Qualifications:- 1 year of prior social work experience preferably in Hospice, Long Term Care, Home Care, or Acute Care environments.
- LMSW required.
- Bachelor's or Master's Degree Required.
- License in TN Required.
- Medicare guideline experience preferred.
- Assess and document patient/family social, emotional, financial, and psychological needs as well as resources on the Comprehensive Assessment.
- Assist patient and/or family with establishing DNR, advance directives, and/or DPOA or Healthcare proxy as requested.
- Assess and arrange patient placement such as Long Term Care, Assisted Living Facilities, and GIP as dictated by patient/family needs.
- Work in both Hospice and Palliative care Programs.
- Be part of a team willing to grow, listen, be heard, and be challenged.
