Be part of a great Team within a great organization – The Social Worker is responsible for providing medical social services to the patient and family in accordance with the physician's plan of care within the guidance of State and Federal Regulations. The role of the SRMC Social Worker is to evaluate patients, determine their needs, and formulate an action plan to ensure that those needs are met efficiently. The SRMC Social Worker will work with all customers throughout the Pole Creek Campus that require assistance, but the primary role is to collaborate in the discharge plan of the hospitalized patient.

Education & Experience:
Education: Masters degree (MSW) in Social Work.
Experience: Prefer at least 2 years of experience in the medical social work field (acute care and geriatrics).
Required licensure/certification: Must have a current and valid license as a Social Worker in the State of Nebraska.
