Purpose and Scope – To provide behavioral healthservices to individuals with severe psychiatric disabilities, that focus onmaintaining individuals in the least restrictive environment possible. Interventions include, but not limited to,assisting individuals:
a. Toidentify treatment needs;
b. Toidentify housing, food/nutritional needs;
c. Toidentify employment/ career needs;
d. Toidentify primary care needs;
e. Tolink with community resources to meet needs;
f. Todevelop daily living skills to meet needs;
g. Tominimize the negative effects of symptoms of behavioral health;
h. Todevelop symptom self-management skills to prevent the need for more intensiveservices;
i. Toincrease social and leisure skills and activities, and develop supportnetworks;
j. Toassist the individual meet identified treatment goals/ objectives.
Duties & Responsibilities
k. Provideservices to individuals in a community-based setting.
l. Utilizeagency vehicle for daily transportation to/from appointments.
m. Locateneeded services and make referrals/ arrangements for treatment and supportservices to meet the individual’s treatment goals / objectives.
n. Ensurethe individuals gain access to needed services by assisting the individual movebetween and among supports and services.
o. Activelyassist the individual to acquire needed resources including income, entitlementbenefits, housing transportation, etc.
p. Implementand organize the delivery of specific social services with their community.
q. Maintaincontact with other social service agencies and health care providers involvedwith individuals to provide information and obtain feedback on individual’soverall progress.
r. Maintainservice productivity of 16 units daily.
s. Collaborativedocumentation.
t. Maintainall clinical documentation according to policy and service provisionexpectations.
u. Maintainall vehicle requirements according to policy.
v. Maintainall IT equipment (ie; cell phone, laptop) according to policy.
w. Followall applicable SBHS Policies.
Organizational Relationships
x. Coordinateswith Clerical staff, Psychiatrist, Nurse Practitioners, Physician Assistants,Registered nurses and Paraprofessional’s to facilitate appropriate servicesbased on individual needs.
y. Amember of and participates in treatment teams and staff meetings.
z. Attendand participate in required supervision.
Minimum Qualifications
High School Diploma or GED. Two years’ experience in a social service-related position. One year at the lower level or position equivalent.
Must have a valid driver’s license with a good driving record. Proficiency in MS Word, Excel, Outlook and Power Point software.
Preferred Qualifications
Bachelor’s Degree in a social service-related field with 2-3 years’ experience. Prior experience in documentation through electronic medical records.
