Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. A. Education
- Associate Degree or Bachelor's Degree in Nursing required.
- Current state licensure as a Registered Nurse.
B. Direct Previous Experience
- Administrative and management experience desired.
- Two years of long-term care experience desired.
- Working knowledge of prescription drugs, therapy practices and bio-psycho-social sciences.
- Remain in good standing with all standards of the Nurse Practice Act.
C. Skills:
- Proficient computer skills in Microsoft Office (Word, Excel) plus the willingness to learn other computer programs if required.
- Ability to work with minimal or close supervision.
- Must conduct all business with a professional manner and with a high level of confidentiality.
- Must be able to supervise large numbers of employees and resident charges.
- Organized and detailed in work performance.
- Excellent technical, assessment, documentation and writing skills.
D. Abilities:
- Work indoors for the majority of the work day (fluorescent lights, HVAC
system, carpeted and tile flooring, normal office noise levels).
- Climb up to two flights of stairs occasionally.
- Lift objects of 25 pounds or less over the head.
- Move objects of 40 pounds or less.
- Ability to work in time-sensitive or stressful situations.
- Sit in a chair / stand for extended periods.
- Respond to medical emergencies in all areas of the campus.
- Assist with resident evacuations.
