As a Hospice Phone Triage Nurse, you will provide specialized care to patients with life-limiting illnesses and support their families during this challenging time. Operating in a telephonic capacity, the Phone Triage Nurse assists with patient needs, provides guidance, and coordinates care with RN Case Manager to ensure optimal comfort and quality of life for hospice patients.
Responsibilities:
- Assess patients' physical and emotional needs and develop individualized care plans
- Implements current nursing practice following a patient assessment and approved plan of care initiated by the RN Case Manager.
-Provide guidance on managing symptoms such as pain, nausea, dyspnea, and anxiety through appropriate interventions and medication adjustments, in collaboration with the hospice interdisciplinary team.
-Collaborates closely with hospice team members, including physicians, nurse practitioners, social workers, spiritual caregivers, and complimentary therapists, to ensure seamless coordination of care and timely response to patient needs
-Offers clear and empathetic education to patients, families, and facilities regarding disease progression, symptom management, medication administration, and end-of-life care options.
-Provides accurate and timely documentation of patient care services to reflect the documented plan of care.
Current LPN degree required; hospice, home care, case management or acute care experience required.
The successful candidate will be skilled in physical, emotional and spiritual symptom management while working in conjunction with an IDT and functioning independently in their role.
