Your Responsibilities as an MDS Coordinator:

In this role, you will be entrusted with critical duties to ensure the highest standards of care for our long-term residents:

• Assess and appraise the caliber of care being administered to long-term residents.
• Initiate comprehensive interviews and evaluations of incoming residents.
• Develop meticulously structured care plans encompassing both short-term and long-term objectives for advancement.
• Spearhead interactions with multidisciplinary treatment teams, caregivers, and families through productive meetings.
• Collaborate with community specialists to garner external expertise in pursuit of care plan goals.
• Participate in Nursing management team meetings and seamlessly interact with all levels of Nursing staff.
• Conduct enlightening in-service education sessions on MDS, RAPS, and Triggers.
• Adhere diligently to the MDS/Care Planning schedule for non-Medicare residents.

What We're Looking For:

Our ideal candidate is armed with the following qualifications and attributes:

• Possess a valid New Jersey RN or LPN license, showcasing your commitment to delivering top-notch care.
• Bring to the table a rich history of 2 years in a Skilled Nursing Facility, underscoring your mastery in this domain.
• Demonstrate a comprehensive understanding of MDS, Medicare, and the care-planning process, illustrating your expertise.
