TITLE: RN Case Manager
REPORTS TO: Director of Nursing
Job Summary:
The RN/Case Manager is a registered nurse, licensed by the state, and has experience in geriatric nursing, rehabilitation, safety, critical thinking skills and team management. Understands the important of being a good leader and team player. Demonstrates understanding and realizing the importance of each staff person in the Clarkfield Care Center team.
Qualifications: Must be registered and licensed with the State of Minnesota as a RN/ and must maintain licensure. The RN Case Manager is also required to have an ability to read, write and speak English and complete and pass a state background study form which checks for certain criminal convictions or serious/recurring maltreatment.
