JOB TITLE/POSITION: Supervisor of Home Care Services

JOB DESCRIPTION SUMMARY
The Supervisor of Home Care Services is responsible for overseeing and monitoring the delivery of home care services to clients, ensuring compliance with regulations, monitoring caregivers’ performance in the client homes, scheduling, recruitment, on call participation, documentation and helping to keep updated client-specific service plans. The supervisor will facilitate conversations between the caregiver(s), the clients, and the internal care team. The supervisor may also communicate with the client's family and/or caseworker(s).
ESSENTIAL JOB FUNCTION/RESPONSIBILITIES
The Supervisor of Home Care Services ensures that the client service plan and requested schedule is effectively implemented by overseeing and monitoring the caregivers’ assigned caseload, scheduled visits, and related activities. The supervisor will also oversee and monitor the clients’ care, which includes, but not limited to, conducting intake assessments and reassessments, monitoring visits, and overseeing compliance related activities. A key responsibility is the ongoing evaluation of care service that requires regular service plan review and monitoring visits as required by the Oregon Health Authority.

Administration, Monitoring, Evaluation, and Communication
- Conduct caregiver interviews, provide orientation, and training.
- Conduct regular home visits to assess service effectiveness and client satisfaction.
- Monitor client's condition(s) and progress, to discover the training needs of caregivers.
- Initiate the service plan in conjunction with the client and/or the client's representative and making necessary revisions as the client's status and needs change.
- Supervise and monitor caregiver to ensure compliance with service plans and regulations, conduct yearly performance evaluations, and provide written documentation that is retained in personnel files.
- Communicate with caregiving team as a part of the service plan development.
Organization and Administrative
- Understand and adhere to Affordable at Home Care Inc., policies, and procedures.
- Create, maintain, and log caregiver schedules and ensure proper staffing for callouts, tardiness, PTO and long-term leaves.
- Review client cases and schedule client visits according to care plans and staff availability.
- Contacts care providers and clients regarding day-to-day changes.
- Maintain caregiver database (e.g., dates and reminders of credentials, training, qualifications, skill set and schedule).
- Facilitate Care Team meetings with caregivers, nurses, and other internal staff to discuss client service plan and share information regarding ongoing care needs and scheduled hours.
- Document client services in automated systems (care logs, care notes, status updates, service plans and updates, client information) or any information that will impact the ability to successfully schedule new caregivers or maintain an ongoing schedule.
- Establish relationships and retain client referral partners such as case workers, families, or 3rd party referrers.
- Maintain documentation of all client encounters and complete reporting requirements according to organization standards in a confidential manner.
- Initiate outreach and missed appointment procedures, as necessary.
- Conduct quality assurance and monitoring activities for service delivery and documentation.
- Gather and report all client related statistics as part of the weekly required KPI review.
- Observe caregivers in the field during in-person visits to assess proper matching.
- Perform general office duties, including, but not limited to word, processing, photocopies, files, shreds, sort/distributes mail, provides reception and telephone services.
Client Caseload Management
- Conduct intake and needs review with client, complete schedule planning, reassessment services and QA monitoring visits as required by the OHA rules.
- Provide day-to-day support, supervision, and performance expectations with each client as needed.
- Provide clinical information to the nurse when medication services or delegated tasks are to be performed.
- Screen clients for eligibility for direct care and support services and when appropriate, and refer clients to needed services, such as mental health, housing, residential care, or skilled care services.
- Take the lead on investigating complaints, incidents and issues and work diligently to resolve each.
Additional Duties
- Participate in the on-call rotation with other Affordable at Home Care, Inc., staff as assigned by management.
- Assume responsibility for personal growth and development and maintain and upgrade professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
- Other duties as assigned, including, but are not limited to, assisting in caregiver interviews, onboarding, incident reporting, caregiving, and training.
WORKING ENVIRONMENT
- Indoor onsite office setting.
- Some local area travel is required.
PHYSICAL REQUIREMENTS
- Ability to participate in physical activity.
- Ability to work for extended periods while sitting, standing and/or being involved in physical activity.
- Moderate lifting of 25 pounds.
- Ability to do moderate bending, lifting, and standing on a regular basis.
MINIMUM POSITION QUALIFICATIONS
- Two years’ experience in the health care field.
- Excellent observation, problem-solving, and communication skills.
- Ability to work independently with minimal supervision.
- Current HCA or CNA certification.
- Documentation of ability to work legally in the U.S.A.
- Valid driver’s license, vehicle, and insurance.
- Preferred: Two years' experience in home care or hospice nursing.
