Patient Safety Manager manages a staff responsible for developing and implementing patient safety programs. Supervises the investigation of errors and then plans for resolution of all safety related incidents, including those involving employees, visitors, patients, and situations posing a threat to the hospital's buildings and equipment. Being a Patient Safety Manager maintains records of patient-related errors and evaluates this information in order to identify the cause of the error. Collects, reviews, and analyzes information from the facility's safety programs to identify corrective action and programs as necessary. Additionally, Patient Safety Manager recommends changes, policies, or programs that could prevent future errors. Designs and presents training programs meant to increase awareness of patient safety initiatives. Ensures compliance with state and federal regulatory requirements. Requires a bachelor's degree. Typically reports to a head of a unit/department. The Patient Safety Manager manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. Extensive knowledge of department processes. To be a Patient Safety Manager typically requires 5 years experience in the related area as an individual contributor. 1 to 3 years supervisory experience may be required.     Patient Safety Manager

    Patient Safety Manager is responsible for collaborating with the Administrative Director of Patient Safety and Risk Management, development and execution of organizational wide strategies to support continuous advancement of a highly reliable safety culture for zero harm to patients.

    Duties:

    • Knowledge of basic components of risk management, patient safety, and quality, including potentially compensable events.
    • Analyzes incident reports and Utilizes the MIDAS database for incident tracking, trending and reporting. Ensures timely completion, submission and accurate leveling of incidents.
    • Communicates appropriately and maintains a good working relationship with physicians, staff, directors, and administrative team. Serves as a resource for Administration, facility staff, and medical staff regarding best practices in safety science.
    • Consults other departments as appropriate to collaborate in patient care and performance improvement activities.
    • Facilitates development of comprehensive, measurable, corrective action plans as a follow - up to safety surveys and communicate actions to staff.
    • Appropriately identifies and forwards reports of incidents (RDEs) and behavior issues to the appropriate medical staff/peer review committee for corrective actions as necessary.
    • Investigates serious incidents, ensuring compliance with UHS Corporate Guidelines.
    • Oversees and responds to any corporate initiative related to patient safety, quality, and/or risk reduction, including Safety Watches, Clinical Risk alerts, and TJC Sentinel Event Alerts.
    • Assists with timely completion and submission of Adverse Event Reports (AER).
    • Identifies undesirable patterns and trends in patient safety and quality outcomes and facilitates the development of action plans to resolve issues in a timely manner.
    • Annually reviews and updates the Risk Management / Patient Safety Plan, including evaluation and design of annual goals and objectives.
    • Leads or actively participates in Root Cause Analysis ensuring that patient safety, quality, and risk reduction strategies are completed in a timely manner.
    • In collaboration with the Administrative Director of Patient Safety and Risk Management, organizes and facilitates the Patient Safety Council, and attends key facility meetings such as, but not limited to Quality Improvement Council, Policy and Procedure Committee, and Fall Prevention Committee.
    • Participates in regulatory survey.
    • Other duties, as assigned

    Qualifications

    Requirements:

    Education requirements: Associate's degree required.

    Work experience requirements: 3 - 5 years clinical hospital experience as a RN. Background in healthcare patient safety, risk management, and/or quality preferred. Knowledge of the operations of the healthcare industry and delivery system. Knowledge of local, state and federal legislation.

    Required licenses/ certifications/courses "Current RN license recognized by the state of South Carolina.

    CPHRM preferred."

    Required skills, knowledge and abilities: Ability to organize and manage; Knowledge of the principles and practices of patient safety and risk management. Basic computer knowledge. Strong written and verbal skills.
