Patient Safety Manager manages a staff responsible for developing and implementing patient safety programs. Supervises the investigation of errors and then plans for resolution of all safety related incidents, including those involving employees, visitors, patients, and situations posing a threat to the hospital's buildings and equipment. Being a Patient Safety Manager maintains records of patient-related errors and evaluates this information in order to identify the cause of the error. Collects, reviews, and analyzes information from the facility's safety programs to identify corrective action and programs as necessary. Additionally, Patient Safety Manager recommends changes, policies, or programs that could prevent future errors. Designs and presents training programs meant to increase awareness of patient safety initiatives. Ensures compliance with state and federal regulatory requirements. Requires a bachelor's degree. Typically reports to a head of a unit/department. The Patient Safety Manager manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. Extensive knowledge of department processes. To be a Patient Safety Manager typically requires 5 years experience in the related area as an individual contributor. 1 to 3 years supervisory experience may be required.     SUMMARY

    Synensys employs patient safety professionals at Air Force Medical Treatment Facilities (MTF) around the world pursuant to a DoD contract in which we have proudly been the incumbent contractor/subcontractor since 2012.

    As a Patient Safety Specialist (PSS) you will work closely with MTF leadership to foster and promote a data-driven culture of safety. You will identify, recommend, and aid in the implementation of process improvements (PI) that facilitate safer care. You will collaborate closely with the Chief of the Medical Staff (SGH), PI staff, Quality Management (QM), and Risk Management (RM) offices. You will be responsible for the coordination of Root Cause Analyses (RCAs), Proactive Risk Assessments (PRAs), and other patient-safety initiatives brought about by the event reporting and proactive risk assessment functions.

    Other duties may include but are not limited to:

    Event and near miss reporting: Provide consultation on how MTFs should use data to identify system and process issues that could have or did result in harm; assist with classifying reported events and advising on what level of assessment is required.

    Education and training activities: Orient and educate MTF staff on key patient safety principles and on elements of a reliable safety culture.

    Foster effective teamwork: Assist with facility-wide implementation of healthcare team training using techniques such as Train-the-Trainer, Train-the-Staff On-site, and awareness training, including TeamSTEPPS.

    MINIMUM QUALIFICATIONS:

    • Bachelor's degree, in a nursing or healthcare related field
    • Four (4) years of experience in healthcare related field; military inpatient facility experience highly desired.
    • Bachelor's level skills in written and oral communications, public speaking, and fosters positive interpersonal relationships including working
    • Proficient skills in Microsoft Office (i.e., Outlook, Word, Excel, and PowerPoint).
    • For PSS personnel with data analytic duties: basic skills in Microsoft Access, Visual Basic, Structured Query Language (SQL), related databases, and visual report design tools. Basic skills in quantitative and qualitative data analysis.
    • The ability to pay attention to detail, work independently, organize time, manage multiple projects simultaneously and the ability to effectively communicate PS concepts to leadership and AFMRA staff.
    • Experience with patient safety aspects of healthcare accreditation is highly desirable. PSSs without such experience must become familiar with PS accreditation requirements.
    • Documented experience with performing RCAs and PRAs. Proficient in applying analytical and evaluative methods and techniques to a broad range of functional areas related to Lean Six Sigma and other commercial quality/performance improvement practices documented.
    • The ability to work collaboratively with Risk Manager (RM) and Quality Manager (QM).
    • DHA Patient Safety Professional Course (PSPC): If new hire has not attended the course within last 10 years, employee must attend next available course.
