Pharmacist - Home Care compounds and dispenses prescribed medications and pharmaceutical supplies per physician's instructions for patients in a home care setting. Provides consultation and answers inquires from patients, families, physicians and other healthcare professionals regarding drugs and their use. Being a Pharmacist - Home Care maintains all required documentation and data on prescriptions, inventory, and patients. Adheres to JCAHO standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Pharmacist - Home Care's years of experience requirement may be unspecified. The Pharmacy Intern is an entry-level position and duties are designed to develop knowledge base and competency level while performing pharmacy functions under the direct supervision of a clinical pharmacist.
JOB DUTIES:
Provides patients with information concerning prices, number of refills available, pharmacy policies and insurance as requested
Facilitates phone calls from patients, family members, other pharmacies, and other members of the health care team as appropriate
Counsel patients on prescription and over-the-counter medications and devices
Apply federal and state legal standards surrounding medication use
Complete all other duties as designated
