Pharmacist - Home Care compounds and dispenses prescribed medications and pharmaceutical supplies per physician's instructions for patients in a home care setting. Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use. Being a Pharmacist - Home Care maintains all required documentation and data on prescriptions, inventory, and patients. Adheres to the Joint Commission standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy. Requires Registered Pharmacist (RPh). Typically reports to a manager. Pharmacist - Home Care's years of experience requirement may be unspecified. Job Description
*Reliable transportation on the job needed
- providing basic personal care as needed
- light housekeeping, changing bed linens and towels, clothing and linens laundry, keeping the house tidy
- help with meal preparation
- help with food shopping (sometimes special diet prescribed for the patient)
- running errands for patients, assisting with correspondence
- companionship
- administering and reminding of oral or topical medications as needed
- assisting with personal grooming, including bathing, dental hygiene, use of bedpans and disposal of bedpan waste as needed,
- assisting with mobility, getting in and out of chairs, bathtubs, beds and vehicles
- assisting in using canes and walkers
- assisting in getting to appointments, especially medical appointments, doctors' offices and other trips outside the home
