Summary of Position: The Oncology Clinical Pharmacist will compound, package, label, and dispense drugs and intravenous admixture solutions. The Clinical Pharmacist will review and confirm accuracy of work performed by technical pharmacy support personnel; review prescribed drug therapy for appropriateness, effectiveness, and safety based on specific data such as age of patient, disease state, and clinical status.
Primary Job Responsibilities: Interprets, reviews and evaluates chemotherapy medication orders for optimal dose, dosage form, frequency, concentration, duration, drug-drug interactions, drug-disease state interactions, drug allergy interactions, and potential cross allergencity, taking into account the individual patient’s special or age related needs. Participates in the dosing and checking of dose calculations for chemotherapy agents.
Compounds, labels, dispenses, handles, and disposes of oncology drugs according to hospital policies and guidelines and regulations established by various regulatory agencies (OSHA,NIOSH, NYS, USP <797>, USP <800>).
Assists with the establishment and maintenance oncology drug preparation regimens in conjunction with physician and nursing staff using nationally recognized guidelines and templates (such as those available from the NCCN)
Maintains oncology drug inventory and oversees storage, record keeping and dispensing of all oncology medications including investigational oncology medications.
Participates and documents quality improvement process within the department, helps maintain continuous Joint Commission compliance.
Provides patient counseling regarding oncology drugs and prescribing regimens.
Qualifications: Graduate of an accredited school of pharmacy. Doctor of Pharmacy preferred. A minimum of two years postgraduate experience in direct patient care in an Oncology setting or an oncology specialty Residency required. Membership and active participation in professional pharmacy and oncology organizations is encouraged. Current license and registration with New York Board of Pharmacy required
Salary Range: $52.92 - $68.67
Pay Grade: CSP
