    The Population Health Data Abstraction Specialist will be responsible for collecting, analyzing, and submitting population health data to support population health initiatives. The ideal candidate will have experience in data abstraction, analysis, and reporting, as well as a strong understanding of population health concepts and methodologies.

    Responsibilities:

    • Review patient records for specific clinical information
    • Retrieve and submit clinical data to meet organizational clinical quality measures
    • Conduct data quality checks and ensure accuracy of data
    • Stay up-to-date with population health trends and best practices

    Requirements:

    • Strong analytical and problem-solving skills
    • Proficiency in data entry with a high level of accuracy and attention to detail
    • Excellent written and verbal communication skills
    • Ability to work independently and as part of a team
    • Proficiency in medical terminology and treatment protocols
    • Proficiency in Microsoft Office
