Position Summary:

Promotes, Coordinates, and assist with both internal and external corporate health services and programming

Responsibilities

• Possesses an in-depth knowledge of the population health management software and utilizes to perform a variety of duties.
• Acts as a subject matter expert about services offered at Covenant Health facilities and shares appropriately with clients.
• Provides service information to both current and potential clients.
• Coordinate both the health risk assessment completion as well as biometric, including lab screenings for large businesses that may have multiple sites or special needs.
• Coordinates education, screening and other events for clients.
• Organizes health fairs, flu shot campaigns and other on-site services as requested.
• Provides health coaching and maintains coaching reports for both internal and external clients.
• Provides professional educational presentations for both internal and external groups.
• Leads group fitness programs and initiatives to help teach basic skills for improving lifestyles or managing disease states.
• Accountable for maintaining medical records/information, and confidentiality of such information for past and present patients
• Responsible for performance and quality improvement activities; works as a team member; attends meetings and participates on committees as needed and requested.
• Establishes positive working relationships with all Covenant and non-Covenant Health customers, serving as a positive role model for Covenant Health.
• Collaborates with the patient, physician and other care team members as part of a team based approach to overall patient care.
• Local travel required.
• Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
• Performs other duties as assigned.

Qualifications

Minimum Education:           

None specified; will accept any combination of formal education and/or prior work experience sufficient to demonstrate possession of the knowledge, skill and ability needed to perform the essential tasks of the job, typically such as would be equivalent to a Bachelor’s degree. Preference may be given to individuals possessing a Bachelor’s degree in a directly-related field from an accredited college or university.

Minimum Experience:         

Two (2) years in health and wellness working with individuals and/or groups in educational, instructional, and/or a consulting roles.

Licensure Requirement:     

Employee must have a valid Tennessee driver’s license and state mandated minimum insurance coverage. Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure.
