    The Population Health Clinical Pharmacy Specialist (CPS) will collaborate with primary care physicians and staff to provide coordinated, comprehensive, high-quality care to PMG patients, focusing on medication-related population health measures. The responsibilities of the Population Health Clinical Pharmacy Specialist predominantly fall into the following areas: (1) patient-centered population-based medication management, (2) program engagement and process improvement, (3) data collection and reporting.

    Required Qualifications:

    • PharmD from an accredited college or school of pharmacy.
    • Coursework/Training Completion of a PGY-1 residency.
    • Coursework/Training Completion of a specialty/PGY-2 residency in ambulatory care Or equivalent educ/experience
    • 3 years experience in primary care pharmacy (or related area) involving direct, collaborative patient care.
    • Upon hire: Oregon Pharmacist License
    • Within 2 years of hire National Certification from Board of Pharmacy Specialties
    • Upon hire: Washington Pharmacist License - based on clinic assignment
    • Experience in writing, including research protocols, medical papers for publication, and educational pieces for patients and healthcare providers.
    • Experience precepting students and/or residents.
