Reporting Relationship: Clinic Manager
General Summary of Responsibilities:
Provides nursing care for patients throughout lifespan in clinics, as directed by physician. Assists physicians with scheduling patient for various diagnosis exams and consults as required to provide continuum of patient care. Main point of contact for clinical initiatives through ACO. Assesses and promotes quality objectives through ongoing audits and reporting. Obtains, reviews and utilizes population health data, risk and cost analyses to affect positive change in ACO beneficiary outcome. Has a significant presence in physician offices implementing new best care practices.
The Population Health RN uses a comprehensive understanding of the care continuum and its intersection within the ambulatory care setting in order to facilitate clinical workflows that improve patient quality, experiences, and decrease costs, ensuring the organization’s vision of population health is seamlessly integrated within the ambulatory clinic. The Population Health RN will assess, plan, coordinate, evaluate, and delegate the delivery of patient-centered nursing care in the primary care setting, in collaboration with clinic leadership, to allow for the standardization of tasks and workflows that support all staff and providers working to the top of their licensure. In partnership with a Clinic Supervisor and Provider Lead, the Population Health RN will be an essential member of the clinic leadership team, providing added clinical nursing expertise and leadership to improve: When directed by the Clinic Manager, Population RN Nurse assumes charge nurse role. Requires verbal skills to communicate in a professional and courteous manner with patients, physicians, various staff members and the general public to provide the highest quality of care.

EDUCATION
Required: Graduate from an approved Licensed Practical Nursing Program
Provides nursing care to patients throughout the lifespan.
EXPERIENCE/QUALIFICATIONS
License/Certifications:
Required: Current and Valid State of Nebraska RN license & BLS for healthcare workers
