Population Health Specialist jobs in Parkersburg, WV

Expected Shift:  Tuesday-Saturday 2pm-10pm

Essential Duties and Responsibilities:

· Monitors general health conditions of Individual including checking vital signs, performing basic medical treatments, and administering medications.

· Order medication under the supervision of Prescriber and according to State Law.

· Verifying physician orders are current, properly documented, and communicated to direct care staff.

· Training clients on individualized medical and/or health needs (e.g., wound care, medically necessary diets, etc.).

· Collect medical data for RNs.

· Obtains informed consent from consumers.

· Consult with RN regarding Individual’s specific issues when a medical need arises.

· Maintains facility to ensure sanitary conditions.

· Complying with federal, state, and company policies, procedures, and regulations.

· Maintaining confidentiality of all Individual information and complies with HIPAA regulations.

· Demonstrates knowledge and support of Westbrook Health Services’ mission, vision, values, policies and procedures, operating instructions, confidentiality standards, and code of ethical behavior.

· Other duties may be assigned to meet business requirements.
