Field RN Case Manager

HealthCare Support is actively seeking a Field RN Case Manager to fill an opening with a Fortune top 50 Healthcare Organization in New Jersey (Passaic and Union Counties).

Daily Responsibilities for Field RN Case Manager:

• Assist the provider/team with various care coordination activities
• Assist Providers in assuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation
• Assist in managing members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
• This position required travel between facilities in Passaic and Union counties.
• Monday – Friday 8:30am-5:00pm EST

• Associate degree or higher in Nursing (RN)
• Active and unrestricted license in New Jersey
• Certified in Basic Life Support
• Minimum 1 year working in a hospital or Long-Term Care facility as an RN
• Full COVID-19 vaccination is an essential requirement of this role
• Access to reliable transportation

Pay Details: $47/hour - $47/hour
