JOB SUMMARY: The Rehabilitation Specialist assists the Agency by providing direct intervention and access to additional resources to clients and their families through professional assessment, service planning, evaluation, individual rehabilitation, case management and advocacy. The Rehabilitation Specialist is self-motivated and remains current on community resources to offer assistance and education to clients and their families. The Rehabilitation Specialist will be responsible for participation in community outreach and promote services offered by Counseling4Kids to prospects, the community, and other professionals. The Rehabilitation Specialist works closely with the therapists and Community Partners to assure that the clients’ needs/case planning are identified and met.
