The Addiction Counselor/Case Manager is responsible for the assessment of mental health needs of participants and provides direct therapeutic services to individuals diagnosed with HIV/AIDS, substance abuse disorders, and co-occurring mental illness and assists with tracking and updating of their medical care. The Addiction Counselor/Case Manager also provides medical case management services to participants of Project Lazarus.
Core Responsibilities
· Provides direct therapeutic services for the residents and participants of Project Lazarus, including group and individual therapy, with a focus on substance use disorder, in a format or modality which is appropriate to the clients’ cognitive abilities.
· Meet with participants at least weekly to provide case management services related to medication management and medical care coordination, as well as linkage to any other community resources as it relates to identified needs on participant service plan.
· Develop therapeutic alliance with participants. Provide therapeutic rapport and support during other participant meetings and gatherings.
· Support the referral and intake process for new participants, in conjunction with the Case Manager.
· Collaborate with the Case Manager in developing a participant-driven service plan addressing housing stability, financial stability, medical and mental health; employment/educational activity and encouraging participant independence.
· Collaborate with the clinical team to maintain a clinically-focused, structured, relevant program for Project Lazarus participants, which includes group and individual therapy, health education and wellness activities, support and modeling of activities of daily living, social and spiritual opportunities, as well as work and educational opportunities.
· Support the maintenance of a cooperative, accountable, inclusive, compassionate, boundaried, autonomous, safe, diverse, and friendly milieu for participants through modeling of these behaviors for participants and staff.
· Support the Resident Care Coordinator in the recruitment, hiring, and training of Direct Support Professionals.
· Lead weekly clinical review meetings.
· Complete in a timely manner case documentation, including assessments, treatment plans, group and individual case notes, service plans, and discharge planning for the purposes of treatment planning, monitoring, and grant reporting and meeting funding requirements.
· Develop and maintain positive working relationships with medical providers, substance use disorder programs, transportation, government agencies, and other applicable social services.
· Knowledge of and compliance with state and national standards of practice, HIPAA, and organizational policies and procedures.
· Attend appropriate and relevant local, state, and national trainings, meetings, and conventions that benefit the Clinical Team and Project Lazarus participants.
· Drive company vehicles if necessary, with and without participants in the vehicle.
· Attend community and/or funder meetings as assigned.
· All other duties as assigned.
