POSITION SUMMARY
Provides relief for care providers and/or family members caring for individuals with physical and/or cognitive impairment. Assists with all individual care needs based on Plan of Care (POC). Services are exclusively intended for the care of the disabled individual not for family members or other individuals. This position works under the supervision of the HCB Waiver Program Coordinator/ES Program Coordinator.
PRINCIPAL DUTIES AND RESPONSIBILITIES
1. Provides intermittent or temporary relief of a primary caregiver’s duty at the consumer’s home, outside the home or in assisted living homes.
2. Undertaking care responsibilities relating to the individuals Plan of Care (POC) during the absence of the primary caregiver.
3. Tasks may include bathing, toileting dressing, feeding, therapy and other duties as needed.
4. Ensures the safety and supervision of the individual.
5. Adhere to confidentiality guidelines, as outlined by organization policy and HIPAA regulations.
6. Some assignments may require over night shifts.
7. Attend and maintain all training requirements in accordance with current applicable Federal, State, and local requirements, as well as organizational policies and procedures.
8. Provides required documentation to the Program Coordinator.
9. Reports any changes or concerns to the Program Coordinator, relating to the individual in which services are being provided to.
MINIMUM REQUIREMENTS
Must be at least 18 years of age. Must provide a criminal history statement and fingerprints, which show there is no history of crimes that would pose a current danger to consumers. Must be self-motivated, dependable, and have the ability to work with minimum supervision. Must obtain CPR and First Aid within 6 months of hire and must keep current. Knowledge of Inupiaq language and culture preferred. Must be able to lift at least 60 pounds. Will be able to demonstrate ability to follow oral and written instructions.
