Quality Assurance Coordinator - Healthcare plans, coordinates, and implements the quality management and quality improvement programs for a healthcare facility.   • Job Summary:

    The Quality Assurance (QA) Coordinator prepares training materials to conduct quality training for personnel. The Quality Assurance (QA) Coordinator prepares for and assists in conducting internal audits to ensure audit information and observations are captured and recorded. This position also provides consultation and direction to ensure programs and services are implemented at the highest standards and patients receive the highest level of care. 

     

    Reports to: Senior Director-North Region

     

    Job Qualifications: 1-2 years of healthcare experience (home health preferred), strong attention to detail, excellent written and verbal communication skills, Ability to sustain efficient, timely, and accurate work in fast-paced work environment. Strong experience with Microsoft Office

     

    Education: High School Diploma or equivalent

     

    Experience: Minimum 2 year experience as QA in the home health care strongly preferred.

     

    Transportation: Reliable transportation. Valid and current auto liability insurance.

     

    Environmental and Working Conditions: Works in patients’ place of residence in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls.

    Physical and Mental Effort: Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient and family individualized psychosocial needs. Requires eye-hand coordination and manual dexterity.

    Essential Functions & Evaluations:

    • Under the direction of the Senior Director, assists in identifying the patient and attendant charts for completion and accuracy
    • Coordinating with field staff to ensure prompt submission of notes within 72 hours
    • QAPI
    • Infection control
    • Emergency preparedness
    • Complaints/Grievances
    • Incident reports
    • Maintaining appropriate records, files, documentation, etc.
    • Assisting in the development of new processes in collaboration with the teams
    • Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
    • Assures the continuity of care through delivery of quality patient care.
    • Monitors assigned cases to ensure compliance with requirements of third party payor.
    • Demonstrates commitment, professional growth, and competency.
    • Aggregates and trends data collected to correct identified problems in collaboration with management.
    • Monitors all service providers for effective interchange and coordination of care in providing care.
    • Prepares annual Quality Improvement report as part of annual evaluations.
    • Demonstrates an in-depth knowledge of, and ensures compliance with, all local, state, and federal laws relating to the operations of the home health.
    • Assists with direction of programs through delegation of responsibility to administrative and supervisory personnel.
    • Assists with establishment of a public relations program for interpretation of the Hospice’s services and to foster good working relations with physicians and community agencies.
    • Carries out other duties as assigned by the Administrator and the Governing Body.
