    Job Summary: To act as a liaison and representative of the residents’ social interests and to ensure that the medically-related emotional and social needs of the residents are met and maintained on an individual basis. The social worker is part of the resident care team and will be responsible for behavior management plans, discharge planning, and review/education on advanced directives and assure resident safety.

    Essential Functions and Responsibilities:

    • Performs resident comprehensive assessments and discharge assessments.
    • Performs MDS assessments and subsequent assessments per State and Federal Regulations (to include PPS assessments, annual assessments, quarterly assessments, and significant change assessments).
    • Coordinates discharge planning
    • Coordinates family meetings and provides updates on resident needs/progress as indicated.
    • Develops resident care plans and behavior management plans and update them as indicated.
    • Writes progress notes at defined intervals according to regulations or more frequently as status requires.
    • Maintains, tracks, and analyzes of grievance log for trends.
    • Reviews and educates advanced directives with residents’ and their families.
    • Attends required meetings ie: care plans, utilization review, CQI, etc.
    • Coordinates resident services for eye care, hearing care, and dental care.
    • Performs assessments for need of psychiatric/psychological referral and follow through.
    • Participates in resident crisis management as indicated.
    • Performs Medicaid application tracking and cares unit submissions.
    • Provides assistance with representative payee forms and submission.
    • Provides education to residents’ and/or their responsible party on Medicare Part D.
    • Provides yearly in-service to all staff on Resident Rights.
    • Meets facility training and in-service requirements.
    • Assures residents safety at all times.

    Other Functions and Responsibilities:

    • Other duties as requested or assigned.

    Working Conditions:

    • Usually works indoors but may have to work outside during outside events and parties.
    • May be exposed to loud noises and sounds that are distracting.
    • May be exposed to diseases or infections from contact with residents or other staff members.
    • May sometimes need to wear safety gear such as gloves and or masks.

    Educational, Physical, and Other Requirements:

    • Must have a Bachelor’s Degree in Social Work or a Bachelor’s Degree in human services field including, but not limited to, Sociology, Special Education, Rehabilitation Counseling and Psychology.
    • One year experience in a health care setting working directly with individuals.
    • Ability to manage time effectively.
    • Must dress professionally and act in a manner that best represents the facility and staff.
    • Must be familiar with facility and village procedures and personnel in order to assist guests, residents, and staff.
    • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, and the public.
    • Ability to follow written and oral instruction.
    • Must be able to effectively communicate with our residents, family members, and staff.
    • Must be able to frequently lift and /or move up to 30 pounds, and occasionally up to 50 pounds.
    • Must be capable of maintaining regular attendance.
    • Must meet all local health regulations and post offer physical exam.
    • Employee must be able to perform the essential functions of the job with or without reasonable accommodations. Physical requirements may include frequent use of hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; talk or hear. The employee may be required to lift, stand; walk; sit; climb or balance; stoop; kneel; crouch or crawl; and smell. Vision abilities may require close vision, distant vision, color vision, peripheral vision, depth perception and the ability to adjust focus.
