Supervisor of Vocational Rehabilitation Services
UBA WeCARE provides a continuum of services to help HRA referred cash assistance clients with medical and/or mental health conditions that affect their employability to attain their maximum levels of self-sufficiency. Individuals referred to WeCARE receive medical and psychosocial assessments to identify possible clinical conditions and social barriers that may affect their ability to participate in work-related activities. Based on the results of the assessments, a determination of an individual's functional capacity outcome (FCO) is completed, and a customized service plan is developed to provide a range of services tailored to meet a client's particular needs.
We are currently seeking an Assistant Director of Vocational Rehabilitation Services
to join the UBA WeCARE program.
Job Responsibilities:
· Assist in the development of Vocational Rehabilitation Services (VRS) Team policies, procedures, and workflows that comply with HRA requirements for the WeCARE Program.
· Work with the Assistant Director of VRS to ensure that cases are assigned appropriately to VRS case managers.
· Review and update daily, weekly and monthly reconciliation reports.
· Work with the Assistant Director of VRS Team to ensure activities lead to timely completions of VRS Service Initiation in SEAMS and appropriate referrals for childcare are done within the expected time frame.
· Ensure VRS case managers have discussions with clients regarding career goals, reasonable accommodations, barriers to employment and prepping clients for employment within the required time frame.
Job Duties:
· Supervise VRS case management staff.
· Monitor all VRS case managers' caseloads.
· Monitor activities assigned to all VRS case managers.
· Evaluate VRS case managers for work performance and oversee corrective actions that may be needed.
· Complete probationary reports and annual performance reviews of assigned VRS staff.
· Ensure VRS case managers are adequately assessing WeCARE clients during service initiation and follow up appointments.
· Direct the VRS Social Worker Assistant/Case Manager to provide outreach to remind clients of pending appointments and attempt to re-engage clients if they have missed an appointment.
· Use clinical skills to diffuse tension and successfully engage clients so that they participate in the VRS engagement process.
· Work with AD of VRS to ensure that VRS milestones are achieved in the time frame required by funder (HRA)
· Ensure that VRS staff are compliant with HRA and Montefiore policies and procedures.
· Associate must have ability to develop detailed policies and procedures.
· Skill managing and directing staff in accessing medical/health/behavioral health diagnoses.
· Skill managing and directing staff in accessing services to address barriers to employment such as obtaining approved forms of government identification, interview clothing, childcare, improving credit rating, etc.
