The Release Service Case Manager will assist CES -families into a “no-wrong-door network” by assessing the needs of families experiencing homelessness and linking them with the most appropriate housing and services to end their homelessness. The goal of the CES-Release Case Manager is to streamline processes through which homeless families assess housing.
The position also provides leadership to other Client Resources Coordinators, II and III and handles more difficult cases in serving clients through counseling, advocacy, and guidance. Release Case Managers are responsible for preforming triage screening, coordinating crisis housing, connecting families to supportive services to address any immediate needs such as emergency shelter, bridge housing, DV services and rapid rehousing services.
