Job Summary:
We are seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will be responsible for overseeing the care and well-being of our residents. Responsibilities:
- Coordinate and oversee the daily care of residents in an assisted living facility
- Conduct assessments to determine the individual needs of each resident
- Develop and implement care plans based on residents' needs and preferences
- Monitor and document residents' vital signs, medications, and overall health status
- Assist residents with activities of daily living (ADLs) such as bathing, dressing, and grooming
- Provide first aid and emergency care as needed
- Ensure that all resident rights are respected and upheld
- Collaborate with healthcare professionals, families, and other staff members to ensure comprehensive care for residents
- Maintain accurate and up-to-date records of resident care
Qualifications:
- LPN with previous experience in caregiving or a related field is preferred
- Knowledge of caregiving techniques, including medication administration and assistance with ADLs
- Familiarity with systems for documenting resident care
- Strong communication skills and the ability to work effectively in a team environment
- Compassionate and patient demeanor when working with residents
- Ability to prioritize tasks and manage time effectively
- Understanding of dementia care principles is a plus
