Being a Resident Care Coordinator communicates with families and medical professionals as needed, and ensures care plans for residents are properly documented and updated. Requires a high school diploma. Additionally, Resident Care Coordinator may require Certified Nursing Assistant (CNA). Typically reports to a manager or head of a unit/department. The Resident Care Coordinator supervises a small group of para-professional staff in an organization characterized by highly transactional or repetitive processes. Contributes to the development of processes and procedures. To be a Resident Care Coordinator typically requires 3 years experience in the related area as an individual contributor. Job Summary
The Resident Care Coordinator (RCC) is a non-professional employee trained to provide personal care and related services to residents in the community according to documented Individual Service Plan(s) ("ISP"). May be a Certified Nurse's Aide but this is not required. The RCC also assists in the training of, supervision and motivation of Resident Care Partners and other ancillary duties as described herein.
