Resident Care Coordinator jobs in Nashua, NH

Resident Care Coordinator coordinates, supervises, and evaluates the care of residents in a skilled nursing, assisted living, or similar facility. Schedules and trains care staff, and ensures that clinical services are delivered in accordance with regulations and professional standards. Being a Resident Care Coordinator communicates with families and medical professionals as needed, and ensures care plans for residents are properly documented and updated. Requires a high school diploma. Additionally, Resident Care Coordinator may require Certified Nursing Assistant (CNA). Typically reports to a manager or head of a unit/department. The Resident Care Coordinator supervises a small group of para-professional staff in an organization characterized by highly transactional or repetitive processes. Contributes to the development of processes and procedures. To be a Resident Care Coordinator typically requires 3 years experience in the related area as an individual contributor.     Job Summary: 

    The MAT Nurse Care Coordinator will assist in the provision of services to individuals with OUD in need of evidence-based MAT alongside necessary outpatient and wraparound services to support recovery. Harbor Care will provide MAT services to the general population, but particularly focused on individuals at-risk or experiencing homelessness, as well as other marginalized populations, in need of additional supports to be successful in recovery. Per SAMHSA, MAT is the use of FDA-approved medication, in combination with counseling and behavioral therapies, to provide a “whole-patient” approach to the treatment of substance use disorders.

    The MAT Nurse Care Coordinator will report to the Director of Nursing and work closely with the Medical Director and other prescribers. The MAT Nurse Care Coordinator will conduct most of his/her activities at the 45 High Street location but may also be required to engage/support clients at other locations.
    The MAT Nurse Care Coordinator is responsible to ensure compliance with all applicable Grant, federal, state, and Harbor Care guidelines.
