Resident Care Coordinator jobs in Renton, WA

Resident Care Coordinator coordinates, supervises, and evaluates the care of residents in a skilled nursing, assisted living, or similar facility. Schedules and trains care staff, and ensures that clinical services are delivered in accordance with regulations and professional standards. Being a Resident Care Coordinator communicates with families and medical professionals as needed, and ensures care plans for residents are properly documented and updated. Requires a high school diploma. Additionally, Resident Care Coordinator may require Certified Nursing Assistant (CNA). Typically reports to a manager or head of a unit/department. The Resident Care Coordinator supervises a small group of para-professional staff in an organization characterized by highly transactional or repetitive processes. Contributes to the development of processes and procedures. To be a Resident Care Coordinator typically requires 3 years experience in the related area as an individual contributor.     The Resident Care Coordinator works as a liaison between residents, resident families, and staff. They will support the Health and Wellness Director by assisting in scheduling managing staff, and effectively assuring that systems are in place to provide quality assisted living services in all areas of resident care. Duties also include problem-solving resident concerns and coordinating care with the caregiver team.

    Responsibilities of the Resident Care Coordinator:

    • Assist with the Caregiver scheduling and resident assignments, working within the department allowances.
    • Coordinate admissions with assistant administrators including supervising move-ins to be sure accommodations are as desired and care is immediately implemented.
    • Complete family/resident admission interviews.
    • Immediately bring prohibited conditions or at-risk residents in need of a higher level of care to the attention of the assistant administrator.
    • Coordinate care planning with home health agencies on-site, working within community policy.
    • Arrange for transportation as desired by the resident.
    • Arrange for resident special needs involving other departments, verifying follow through.
    • Function as a liaison with families ensuring special needs/requests/complaints are addressed.
    • Read all communication notes regarding the community between the Caregiver shifts.
    • Assist with coordinating staff training and in-service schedules with the Assistant Administrator
    • Assist with the supervision of the medication room and orders, working with Medication Aides and Caregivers

    If you have these qualifications, we'd love to chat:

    • Positive team player attitude and love working with people!
    • A.A. Degree in Health Care or a related field from an accredited college is preferred.
    • CNA/HCA/ Medtech License Required per State Regulations.
    • A minimum of 2 years of relevant experience in a supervisory role.
    • Basic typing skills along with a working knowledge of personal computing and word processing software are required, preferably in a Microsoft Windows environment.
