Resident Care Coordinator coordinates, supervises, and evaluates the care of residents in a skilled nursing, assisted living, or similar facility. Schedules and trains care staff, and ensures that clinical services are delivered in accordance with regulations and professional standards. Being a Resident Care Coordinator communicates with families and medical professionals as needed, and ensures care plans for residents are properly documented and updated. Requires a high school diploma. Additionally, Resident Care Coordinator may require Certified Nursing Assistant (CNA). Typically reports to a manager or head of a unit/department. The Resident Care Coordinator supervises a small group of para-professional staff in an organization characterized by highly transactional or repetitive processes. Contributes to the development of processes and procedures. To be a Resident Care Coordinator typically requires 3 years experience in the related area as an individual contributor. The Resident Service Coordinator must be highly organized; proficient in technology; work well with people of all ethnicities and socioeconomic backgrounds;
demonstrate a high level of energy; and have the ability to engage clients in services, manage outcomes, and solicit feedback .
They must also be able to take initiative and use sound judgment. Qualifications : Two(2) years of relevant experience coordinating services High school diploma required, college education preferred.
Excellent organizational, computer skills, and customer skills required. Demonstrated ability to organize self and others;
to work independently and to take initiative; to prioritize and to proceed logically. Ability to maintain confidentiality.
Ability to organize large meetings / functions as well as educate the general public regarding Lucy’s Hearth’s policies and services.
