Resident Care Coordinator coordinates, supervises, and evaluates the care of residents in a skilled nursing, assisted living, or similar facility. Summary
The Resident Care Coordiantor is responsible for the daily functioning of the health care needs of the residents in the Community. This includes supervising the Caregivers, training of new staff, and ensure that all staff has continued education at a minimum of 16 hours. This position also includes supervising the Medication Technicians, supervising the medication delivery system, training of new staff, and in servicing needs of existing staff.

Ensure that Caregiver skills check list and pre-service orientation is completed with 30 days of hire.
Review and updates 24-hour logs as needed.
Reviews and updates all ISPS as needed.
Conduct daily staff huddles.
Reviews and documents daily in the communication log as appropriate.
Reviews cleanliness and organization of the community.
Reviews, supervises, and assists with resident care as needed.
Assists residents with meals as needed.
Investigate incidents as they occur and update incident reports in Blue Step.
Reviews and documents on the service plan as needed, assists with 30,60,60-day updates.
Attend service plans review as needed.
Assist and/or conduct the orientation for new employees.
Assist with recruiting, hiring new staff as well as daily management of staff’s performance. (This included performance evaluation, corrective action, and terminations.)
Assist with recruiting and hiring new staff.
Monitors and tracks residents’ weights per policy.
Schedule the exit-training interview with new employees.
Ensure care staff are wearing approved uniforms.
Take on call for staffing and scheduling needs as needed or requested.
Coordinates and plans 2 in services monthly with care staff.
Maintains adequate attendance record, notifies supervisor if late or unable to work in accordance with the personnel policies.
Review and completes the weekly QA requirements in Blue Step which includes reviewing the following:
o Review daily shower sheets, report skin issues to Nurse
o Review daily task sheets.
o Staff Huddles
o Conduct daily floor rounds to ensure care needs are being met.
o Ensure staffing ratios and staffing plans are being followed.
o Ensure that the daily shower schedule is being followed – and follow up with any refusals.
o Ensure that the daily housekeeping schedule is being followed – and follow up with any housekeeping concerns.
o Ensure that the overall culture of community is being modeled through positive direction and coaching and fostering a community culture of kindness.
Responsible for completing quarterly Caregiver skills observation checklists.
Performs all other duties as assigned by any member of management.
Reviews and documents daily in the communication log as appropriate.
Reviews new orders and MAR as directed.
Supervises and reviews the transcription process, medication and equipment ordering, pharmacy cycle fill.
Performs resident care coordination and communication with outside agencies and medical providers.
Reviews alert charting as directed.
Reviews incident reports as needed and completes investigations.
Reviews cleanliness and organization of medication room.
Performs medication pass as needed.
Reviews and responds to Community R.N. and LPN audits to ensure compliance.
Performs all other duties as assigned by ED, LPN, and RN.
Assure that daily med tech assignment sheets are completed.
Assuring that all QA is completed per policy which includes
o Med room audits
o MAR Narc Audits
o Med tech daily checklist sign off
o Acuity report monthly updates
o Med Tech skills observations
o Weekly RSM QA
Acts as the person in charge in the absence of the Executive Director and other managers/department heads.
Minimum Qualifications and Skills
Ability to utilize and apply basic management skills.

Caregiver certification required
Tuberculosis testing is required prior to initial training
Education and Experience
Previous work with the elderly population is desirable. Previous work as a medication assistant is desirable.
Maintains CPR certification.
Universal Precautions
Exposure to blood/bodily fluids is likely.
Physical Requirements
Has the ability to do heavy lifting. (Up to 40 pounds).
