SAP Basis Consultant provides technical support and leadership on SAP Basis systems including establishing standards and requirements, evaluating and directing enhancements, and implementing solutions for performance monitoring, and systems configuration, design and implementation. Designs and develops SAP architecture requirements and specifications to ensure the system is meeting corporate needs. Being a SAP Basis Consultant requires a bachelor's degree in area of specialty. Typically reports to a manager or head of a unit/department. To be a SAP Basis Consultant typically requires 10+ years of related experience. Works on advanced, complex technical projects or business issues requiring state of the art technical or industry knowledge. Works autonomously. We are seeking a dedicated and compassionate Care Manager to join our team. The ideal candidate will be responsible for providing comprehensive support and guidance to individuals in need of care, ensuring they receive the necessary services to improve their quality of life. - Conduct assessments to determine the needs and preferences of clients.
- Develop and implement individualized care plans that address the specific needs of each client.
- Provide crisis intervention and management support as needed.
- Facilitate access to community resources, including Section 8 housing and addiction counseling services.
- Monitor client progress and adjust care plans accordingly.
- Collaborate with healthcare providers, social workers, and other professionals to ensure comprehensive care delivery.
- Supervise and mentor junior staff or volunteers involved in client care.
- Utilize motivational interviewing techniques to empower clients in their care journey.
- Maintain accurate documentation of client interactions and care plans in compliance with regulatory standards.
Experience
- A background in social work or a related field is preferred.
- Experience in crisis intervention and management is essential.
- Familiarity with Medicare processes and regulations is a plus.
- Previous experience in childcare or supervising staff is beneficial.
- Skills in addiction counseling, mentoring, and motivational interviewing will be highly regarded.
