Summary of Duties and Responsibilities:
Under the general supervision of the Director of Home and Community Based Services, the Assessor is responsible to assess and coordinate an initial care plan of home care services or available community resources with long-term care clients and their informal/formal providers within designated cost caps. Assessors assure that the initial services are appropriate and meet the needs of the client in order to safely maintain the client in a community/home setting.
Under the My Care Ohio Waiver, Level of Care (LOC) Assessors will conduct initial LOC’s as well as LOC re-determinations and change in condition for all members participating in My Care through the Managed Care Organization Plans (MCOPs). The LOC Assessor will utilize the ICDS assessment tool specific for My Care, to provide a limited, detailed assessment in order to make a LOC determination.

Minimum Job Qualifications:
Applicant must be a Registered Nurse or Licensed Social Worker with an unencumbered license at least one-year prior experience in home health care, medical social work and/or geriatrics. Applicant shall possess the knowledge and skills necessary to provide the level of assessment required by the client and by any legal regulations. Applicant shall have an understanding of 1.) Medicare, Medicaid and third-party reimbursement and 2.) Community service delivery systems and the ability to utilize the community's resources.
Applicant must possess a valid driver’s license and proof of automobile insurance.
Must also pass a criminal background check and drug screen.
