Social Services Director - Nursing Home in a nursing home environment, directs, establishes, and plans the overall policies and goals for the social services department. Responsible for patient assessments, care planning and helping the patients and families adjust to their new surroundings or occupations.   • General Statement of Job Function: As a member of the facility’s interdisciplinary teams this position has responsibility for the residents’ psychosocial well-being.

    Evaluation of Job Function, Responsibilities, and Expectations:

    • Demonstrates the knowledge of and ability to honor the rights, dignity and confidentiality of residents, staff and facility information and property.
    • Demonstrates excellent communication skills with residents, families, staff and community.
    • Demonstrates ability to document data that meets the regulatory requirements of the department and also provides the care giving staff a frame work to work from. Keeping department in compliance with all regulatory standards at all times.
    • Communicates with all caregivers including medical care providers. Is able to provide observational feedback to assist with adjustments to care approaches and interventions.
    • Participates in Interdisciplinary care planning and the PASSAR, MDS andBehavior Managment process with timely assessment and attendance at meetings.
    • Demonstrates the ability to maintain self and staff current with in servicing including mandatory classes including Infection Control, Fire and Disaster. Is able to appropriately use personal protective equipment.
    • Demonstrates ability to be an active member of resident care committees that assess the events and needs of the resident and then provide programs or systems to meet needs. I.e.: Fall, restraint, behavior, and safety committees as example.
    • Develops; with the interdisciplinary team and resident; all discharge plans including transportation, equipment etc. Making sure the discharge is as smooth as possible for the resident.
    • Demonstrates the ability to maintain community information that assists residents and families with placement issues and home resources upon discharge.

    Education

    • Applicant must be a licensed social worker (LSW/MSW/LCSW)

    Job Type: Full-time

    Pay: $55,000.00 - $80,000.00 per year
